The main purpose of the game is to make it to the finish line with the truck's cargo. Players are given a set amount of time, but can ram into special vans that will add three seconds to the timer. There are several characters to choose from, each with a unique truck and attributes.
The game starts out in Key West and players travel across the United States, ending in San Francisco. After Stage 1, the game gives the player a choice of trailer. One trailer is harder to haul, but provides a bigger payoff while the other choice is easier to haul but provides a smaller payoff. Money is deducted from the total when the trailer is hit. Players can sound the truck's horn to make other cars on the road yield and slipstream behind large vehicles to gain a momentary speed boost.
In addition to the time limit, players also compete with the "Lizard Tail", a rival trucker. Crossing the finish line before the Lizard Tail yields additional money. In between levels, players can park the truck in a mini game to earn more cash.
San Francisco Rush 2049 is the third game in the Rush series, sequel to San Francisco Rush and Rush 2: Extreme Racing USA.
The game features a futuristic representation of San Francisco and an arcade-style physics engine. It also features a multiplayer mode for up to four players and Rumble Pak support on the Nintendo 64 port. A major difference in game play compared to predecessors in the series is the ability to extend wings from the cars in midair and glide. As with previous titles in the franchise, Rush 2049 features a stunt mode in which the player scores points for complex mid-air maneuvers and successful landings. There is also a multiplayer deathmatch battle mode. There are six race tracks, four stunt arenas, eight battle arenas, and one unlockable obstacle course named The Gauntlet. The single player race mode places emphasis on outlandish and death-defying shortcuts in each track. The game has a soundtrack mostly comprising techno music.
The classic PC shooting game has been adapted for tournament play in the arcade. This prototype features modified deathmatch levels and a special custom weapon. Machines may be linked to provide competitive play.

The game was a first person perspective from inside your aircraft's cockpit showing enemy airplanes as green outlines ahead of you that you would target and shoot down. Control of your own aircraft was done using gyroscopic motion controls and the buttons found on the outside top of the handheld's body shape. A radar in the lower left corner was used to determine which direction you needed to turn to find a new enemy aircraft before you were eliminated.
